From defect to closed corrective action: closing the loop
A defect found is only useful if the cause is understood and the fix is verified. Most systems lose the thread in the middle.
Logging a defect is the easy part. What matters is what happens next: who contains it, who finds the root cause, who owns the corrective action, and who checks that it worked.
Where the loop breaks
In spreadsheet-based tracking, the defect log, the 8D or root-cause report and the corrective action list are different files. A defect gets closed because the part was reworked, while the cause is never recorded. The same defect returns three months later.
A closed loop in one record
- The defect is logged with its evidence and a picture of the part.
- Containment and root cause are recorded against the same defect.
- A corrective action is assigned with a due date and an owner.
- Closure needs verification, and a repeat defect is flagged against the earlier one.
The aim is simple: every defect leaves a trail you can follow from the first report to the verified fix, and repeat problems become visible instead of buried.
